How to Send us a Sample
What's a Sample?
A sample is material from one identifiable location. Keep each sample separate. Avoid one contaminating another.
A sample of suspected asbestos material is about one cubic inch of insulation, ceiling texture, or similar "loose" material. Be sure it is representative of the area in question. A sample of flooring material is a piece 2x2 inches with the mastic that adheres to it.
A sample of suspected lead paint is about the amount that covered an area of 2x2 inches. Try not to include the underlying material or substrate.
Packaging a Sample
Asbestos samples should be placed in sealed packets such as plastic zipper storage bags. Then place the first sealed bag in a second sealed one. When shipping in an envelope, protect the sample package with cardboard or plastic foam or bubble wrapping.
Lead paint samples should be placed in a rigid or hard container, such as a photo film canister, coin collector's tube, or spice container — very well washed. Tape the top closed.
Clearly mark each sample with a specific location name and/or number so that you will know which it is in the report.
Filling out a Submission Form
Download a Sample Submission Form and print it. (See the bottom of our home page for this form.)
Fill in the form:
- Use your personal name for "Company." Use the address to which the report should be sent.
- "Contact" is you or someone else who should receive the report or who is the one that we can call for additional information. List the phone and fax numbers for this contact person.
- "Job number" is your identification of the job. "P.O.#" should be left blank
- "Analysis Requested" Just check asbestos and/or lead. You can be more specific if you know the kind of test needed.
- "Reporting Method" Tell us how you want to receive the report about your sample(s).
- "Send Invoice" Check prepaid and include a check or money order. If you have an established account with us you may check another way.
- "Turnaround" Use this only if you want a report in less than 3-5 days after we receive the sample — at extra cost.
- In the boxes, fill in the sample identification (same as mark used on sample package); the date/time of sample collection; the location where it was collected — such as kitchen ceiling, heating conduit-front, nursery window sill-north; and description such as floor tile, insulation, wall paint. If you include both lead and asbestos samples in one shipment, include the word asbestos or lead in each description.
- Sign the form in the "Relinquished By:" box.
- Make and keep a copy of the completed Submission Form for your records.
